**Join Our Team as a Case Holding Supervisor**

Looking for an opportunity to make a real difference in families' lives? We're seeking a passionate individual to join us as a Case Holding Supervisor. In this role, you'll work closely with our Operations Manager for the Outreach Division and support our dedicated team of Family Wraparound Coordinators, Mentors, and Volunteers.

**In your new role you will**
- Conduct Initial calls on referrals received, acting as the Great Minds Together ‘Front Door’.
- Ensure consent and other documentation is obtained and stored securely via Document Vault on CPOMS.
- Create a child’s needs assessment and intervention plan in collaboration with the young person, family and agencies.
- Provide support to families of children and young people with SEND / SEMH needs.
- Liaising with other agencies e.g., school, social care, health, and local authorities to ensure appropriate care planning is in place and SMART planning is effective.
- Liaise with other services and professionals about families where there are complex situations to support the volunteers & wider team.
- Provide regular case updates to your Direct Line Manager.
- Provide written reports when required both internally to management and externally for other agencies.

**Essential Must-Haves**
- Professional experience working in the SEND and/or SEMH sector.
- Experience of managing/ overseeing a team of staff.
- Awareness and experience of the education system, SEND support and the social care sector including social workers, edge of care and looked after children.
- Full UK driving licence and access to a vehicle with business insurance.

**Embracing the Unexpected**

Due to the unpredictable and varied nature of Great Minds Together's work, staff should be flexible, willing to work weekends and evenings as needed. Additional duties, usually compatible with regular tasks, may arise to cover unforeseen circumstances or changes. If these tasks become regular, they'll be added to the job description after consultation with the staff member.

**Job Types**: Full-time, Permanent

Pay: £28,000.00-£31,000.00 per year
