Major Trauma School Reintegration Facilitator

This is an exciting opportunity for an Allied Health Professional or Registered Children’s Nurse to join the Major Trauma Service on a permanent basis to develop their knowledge and skills in supporting severely injured children to transition back into education.
Joining an innovative service you will lead with service improvements and innovations to support the rehabilitation of young people across the hospital and Greater Manchester. You will share learning locally and nationally through the Major Trauma Networks.
The post holder will be a key member of the Major Trauma Multi Disciplinary Team, acting as resource providing education, advice and support to staff and families. You will develop strong working relationships across all clinical areas, education, social care and community health services.
You will act as a key worker for children, young people and their families following discharge, linking in with schools, social care and clinical teams both in the hospital and community to ensure the child/young person returns to school in a timely manner with the reasonable adjustments they need.
You will also support the psychosocial needs of the child, young person and their family identifying barriers to rehabilitation and return to school. You will make ongoing referrals to services in hospital and community to ensure the holistic needs of the family are met.
You will be expected to support the major trauma multidisciplinary team trauma clinic, support service improvements and assist with data collection as part of the role to support improved outcomes for our patients.
Your main base will be Royal Manchester Children's Hospital. The post holder will be expected to have good time management skills and be expected to manage their own caseload of patients. You will work across home, hospital, community and educational settings. The role will involve travelling across Greater Manchester.
