Cancer Patient Pathway Co-ordinator

The Cancer Services Team are looking for a Cancer Patient Pathway Co-ordinator. The Cancer Patient Pathway Coordinator (PPC) will work alongside the Cancer Team to ensure patients have a timely, efficient, effective and supported journey through their cancer pathway.
The role of a PPC (Patient Pathway Co-ordinator) is to manage the cancer patient’s pathway, by working with all members of the MDT(multi-disciplinary team). The post holder will ensure all patients are discussed at the MDM (Multi Disciplinary Meeting) (up to 5 meetings a week) by organising the MDM list and ensuring all patient and clinical information is available for clinicians to make informed treatment decisions. The PPC must record all treatment decisions and ensure all treatment decisions are followed up, by organising new appointment or tertiary referrals.
The PPC will monitor and help ensure the MDT meets all the measures set out in the NHS Cancer Plan and Cancer Peer Review Measure, in particular the cancer 2 week, 1 month and 2 month waiting times. The PPC will work with the lead consultant to develop a service improvement program and implement any actions

The main role of the PPC is to ensure cancer patients pathways have the shortest waiting times possible from referral to diagnosis to treatment. This includes providing administrative support for the Cancer MDM by producing and circulating lists of cancer patients to be discussed, from at least 2 different clinical departments, pathology and radiology. To be responsible that the patient details on the list are correct. This will involve communication with admin staff, registrars and consultants. The number of patients on the list can vary from tumour site to tumour site, but there could be up to 70 patients on 1 MDT meeting.
There is no standard pathway of patients for each tumour site for cancer treatment and the PPC must plan and manage this process.
