About the Role

The Rehabilitation Pathway Coordinator will be responsible for coordinating the rehabilitation patient pathway from admission to discharge, working closely with a multidisciplinary team to ensure seamless care.

With expertise in complex rehabilitation needs, the successful candidate will work across the organization to co-ordinate the care pathway of patients who require specialist rehabilitation, including coordinating care while in the acute setting and facilitating transfers to interim facilities.

This role requires strong communication and collaboration skills, with the ability to work effectively with multiple stakeholders, including patients, families, and healthcare professionals.

The estimated salary for this position is £46,148 - £52,809 per annum, dependent on experience.
