The duties of the post include:

- Undertaking assessments, support planning and reviews.
- Multi agency work and close partnership work with Customers, Carers and other professionals.
- Supporting the Team Manager.
- Supervision and mentoring of staff.
- Excellent communication and social care assessment skills, plus a solid understanding of the relevant statutory frameworks, and best practice.
- Proven experience within a busy social work team and the ability to meet targets and deadlines, including performance management.
- Experience of supporting and motivating team members, supervisory experience would be preferable.
- Working knowledge of Care Act, MHA and MCA.
- Working knowledge and understanding of the Transforming Care Agenda.
- The desire and motivation to work closely with health partners and other professionals.
- Two years relevant post qualifying experience and be SWE registered.
