Patient Pathway Navigator

The post holder will be responsible for providing administrative services to the multi-disciplinary team, assisting the Medical Secretaries, Nurse Specialists and liaising closely with the clinical teams to ensure that all patients under the care of follow-up services within Cardiology genetic services receive their tests and appointments in a timely manner. You will work both independently and in support of the Adult Congenital Heart Disease (ACHD) and Inherited Cardiac Conditions (ICC) services to collect and provide information required by the clinical team to agree a patient’s treatment plan. You will take responsibility for your own workload but will escalate issues to the Directorate team as and when appropriate. You will work within local and national guidelines e.g. national waiting times, national peer review, network agreed referral protocols and internal escalation policy.
Administrative duties including liaising with patients in a face-to-face and via telephone. Dealing with patients in a professional manner.
Deal sensitively with patients and carers.
Sorting and distribution of incoming mail both electronic and paper form
Routine processing of letters, reports and materials ensuring Trust and departmental deadlines are met.
Ensuring urgent matters are brought to the attention of the appropriate person in a timely fashion reporting any problems or issues to senior staff and seeking clarification if unsure of procedure/s.
Ensuring clinics are booked.
Responsible for the timely processing of referrals and that documentation is ready for the triage process
