To be committed to addressing Health Inequalities. To champion the work undertaken in the PCNs around Health Inequalities. To lead the Health Inequalities work for the Federation & support the delivery of the Health Inequalities Directed Enhanced Service specification including the proactive social prescribing element. Directly line manage services which are relevant and focused on supporting addressing health inequalities Work with supporting the Transformation Team on projects which have a focus on addressing health inequalities To support the PCNs to identify patients who may struggle accessing the care they need.

Focusing on patients with mental health and learning difficulties or those groups experiencing health inequalities in access to care. Liaising with PCN staff and facilitate sign posting to services provided by voluntary and community groups linking into developing community assets To be responsible for ensuring community engagement work takes place ensuring aims and objectives are met. To support the PCNs team to engage with wider system strategies ensuring that health inequalities are being addressed. National level: to engage and share local approaches to address health inequalities (as well as Membership of the national Health Inequalities Improvement Forum and access to NHS Future Collaborative Platform) System level: to represent the general practice neighbourhood and community partners in the ICS inequalities improvement agenda.

**Place level**: supporting the integration of primary care, secondary and community services, in collaboration with peer organisational HI Leads, whilst driving the change to help mitigate inequality. Engaging with other place
- based partnerships. Neighbourhood / Community Level: to build collaborative, sustainable and positive relationships with external partners to develop a shared vision and understanding of the issues and challenges and how to work together to meet the needs of the community ensuring that it is from a population perspective. Bring creative and using meaningful ways of supporting our preventative and proactive approach.

To oversee data sharing between the Health and Wellbeing Team, PCNs, Federation and sources such as Population Health Management and Health Inequalities systems to keep the team up to date with the latest data and knowledge. To progress tackling neighbourhood health inequalities service through empowerment and leadership. To strategize for resources and more Additional Role Reimbursement Scheme (ARRS) recruitment, to enable the continuing support of the Health Inequalities work. To ensure clear communication between each PCN to share experience and learning - further enhancing the approaches for each programme.

Support internal and external meetings, representing the PCNs to update on the Health Inequalities programme. Spread the learning and capture primary care & community engagement Work with other system partners on a population perspective.
