Job summary

The advancednurse practitioner will be responsible for a number of clinical areas such ashealth promotion, chronic disease management, health prevention, as well assupporting the PCN leads and management team in the reviewing of clinicalpolicy, practice and procedure with respect to the PCN enhanced access and hubservices.

Apart fromthese clinical responsibilities, you will also work as an autonomouspractitioner, responsible for the delivery of practice nursing services for thePCN and where required within individual PCN member GP surgeries, working aspart of the PCN multidisciplinary team, delivering care within their scope ofpractice to the entitled patient population. You will be a healthcareprofessional NMC/GPC/HCPC registered with a license to practice, be anindependent prescriber.

Main duties of the job

We are lookingfor an Advanced Clinical Practitioner Nurse who must;

have amasters degree level in the relevant area of expertise;

beworking at a masters level aware or equivalent that encompasses the fourpillars of clinical practice, leadership and management, education andresearch, with demonstration of core capabilities and area specific clinicalcompetencies

haveeither graduated from a Centre for Advancing Practice accredited MSc advancedpractice programme or completed the Centres ePortfolio (supported) Route; bothpathways enable eligibility for an Advanced digital badge issued by theCentre, demonstrating recognised educational and experiential preparation inadvanced practice

havecurrent NMC registration.

NLPCN iscommitted to tackling health inequality as a priority. We have developed aprogram of work to improve trust and engage with our local community, trainingour GP surgeries and staff to improve access for patients, improving ourcommunication to patients about service changes and health promotion andimproving the monitoring of those suffering from health inequality to drive ourstrategy.

The Role

We are looking to recruit to the post of AdvancedClinical Practitioner Nurse, to work within our Primary Care Networkmultidisciplinary healthcare team.

Main Role Responsibilities

The following are the core responsibilities. However,there may, on occasion, be a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels.

Clinical Care Delivery

Provide a first point of contact within thePCN/practice for patients presenting with undifferentiated, undiagnosedproblems, making use of skills in history taking, physical examination,problem-solving and clinical decision making, to establish a diagnosis andmanagement plan

Make professionally autonomous decisionsfor which you are accountable

Instigate necessary invasive and non-invasivediagnostic tests or investigation and interpret findings/reports

To prescribe safe, effective andappropriate medication as defined by current legislative framework

Provide safe, evidence-based, costeffective, individualised patient care

Offer a holistic service to patients andtheir families, developing where appropriate an on-going plan of care/support,with an emphasis on prevention and self-care

Promote health and well-being through theuse of health promotion, health education, screening and therapeuticcommunication skills

Actively take a personalised care approachand population centred care approach to enable shared decision making with thepresenting person

Refer patients directly to other serviceswithin or outside the PCN, and agencies as appropriate

Participate in developing and deliveringspecialist primary care services such as long-term disease management(diabetes, asthma, chronic obstructive pulmonary disease, coronary heartdisease and hypertension).

Work with nursing, medical and health careassistant colleagues across the PCN to ensure that National Service Frameworks( Coronary Heart Disease/Older People/Diabetes/Mental Health) are beingdelivered

Work with the PCN multi-disciplinary teamsas well as those within the PCN member GP practices and across the wider CIC,to promote integrated and seamless pathways of care

Contribute to the PCN achieving its qualitytargets to sustain the high standards of patient care and service delivery

Participate in identification of communityhealth needs and develop patient/family-centred strategies to address them inliaison with appropriate community health professionals and agencies

Be aware of and contribute to, planning anddelivery of practice-based commissioning

This is not an exhaustive list and may beamended dependent on staff competency and service need

ANPs cannot provide care topregnant women if this involves assessment of the pregnancy, unless they arealso a practicing midwife meeting the NMC requirements of registration .

Professional role

Promote evidence-based practice through theuse of the latest research-based guidelines and the development ofpractice-based research

Monitor the effectiveness of their ownclinical practice through the quality assurance strategies such as the use ofaudit and peer review

Maintain their professional registration

Participate in continuing professionaldevelopment opportunities to ensure that up-to-date evidence-based knowledgeand competence in all aspects of the role is maintained

Work within the latest NMC Code ofProfessional Conduct

Record accurate consultation data inpatients records in accordance with the latest NMC guidance and other pertinentstandards

Keep up to date with pertinenthealth-related policy and work with the practice team to consider the impactand strategies for implementation

Pro-actively promote the role of theAdvanced Nurse Practitioner within the PCN Community ward and externally to keystakeholders and agencies

Participate in multi-disciplinary protocoland patient group directions development

Identify appropriate opportunities to delegateboth clinical and administrative tasks to more junior staff

Help the PCN operate in a cost effectivemanner

Identify and manage nursing care risks on acontinuing basis

Participate in PCN meetings

Participate in audits and inspections asappropriate

Work closely with the clinical leads andadministrative managers in the setting up and/or improving of systems formonitoring/measuring performance against the indicators for the domains andareas outlined in the current IIF

Work with the PCN member GP practices toassist them meet Clinical Governance and Quality Indicator targets

Ensure that all PCN policies are fullyimplemented

Work collaboratively with colleagues withinand external to the PCN

Participate in audits and inspections asappropriate

Current RGN Registration Membership of a professional body RCN Advanced Nurse Practitioner Degree or BSc in Emergency Care (or equivalent) Independent Non- Medical Prescriber Qualification (V300)
