The ideal candidate would be able to work across threepractices. You will be working alongside other ANPs with the support of the GPteam.

Main duties of the job

This role involves autonomouslymanaging patient list. The role includes autonomously managing your owncaseload. The role focus's on minor illness and urgent, on the dayappointments. As a result,you should hold a prescribing qualification.

As an Advanced NursePractitioner with independent prescribing, you'll deliver face-to-faceservices, enabling patients to obtain health information from the appropriatehealth care professional. You will be providing safe and effective healthassessments and treatment of patients within the establishment.

You'll be accountable for yourpractice including the assessment, diagnosis, treatment, and discharge orreferral of patients, independent of medical intervention.

Working as part of themulti-disciplinary team in providing quality, evidence-based health care tomeet the immediate needs of the patient, you'll take an active role within thecross-disciplinary, multi-skilled team to ensure service quality in line withcorporate goals and strategy; to assist in the implementation of the wellbeingapproach which has a positive impact on the health economy.

About us

Leyton Collaborative PrimaryCare Networks (PCN) was created in July 2019 and comprise a 3 GP surgeries thatserve a neighbourhood of typically between 30,000 and 50,000 residents. The PCNserves a population of approximately 38,000 patients across a varied geography.Post holders will be required to work across 3 surgeries within the PCN.

3 separate practices that haveformed a network in order to benefit from shared resources and to be able tooffer healthcare services at scale to its population.

To be responsible for thedelivery of practice nursing services, working as part of the practicemultidisciplinary team, delivering care within their scope of practice to theentitled patient population. This role will be responsible for a numberof clinical areas such as health promotion, chronic disease management, healthprevention, cervical cytology, immunisations, as well as supporting themanagement team in the reviewing of clinical policy and procedure.

Induction Training

On arrival at the practice allpersonnel are to complete a practice induction programme; this is managed bythe Practice Manager of The Manor Practice.

Learning and Development

The effective use of trainingand development is fundamental in ensuring that all staff are equipped with theappropriate skills, knowledge, attitude and competences to perform their role.All staff will be required to partake and complete mandatory training asdirected by the training coordinator, as well as participating in the practicetraining programme. Staff will also be permitted (subject to approval) toundertake external training courses which will enhance their knowledge andskills, progress their career and ultimately, enable them to improve processesand service delivery.

Collaborative Working

All staff are to recognise thesignificance of collaborative working. Teamwork is essential inmultidisciplinary environments. Effective communication is essential and allstaff must ensure they communicate in a manner which enables the sharing ofinformation in an appropriate manner.

Primary Responsibilities

The following are the coreresponsibilities of the nurse practitioner. There may be on occasion, arequirement to carry out other tasks; this will be dependent upon factors suchas workload and staffing levels:

a. Develop, implement and embedhealth promotion and well-being programmes

b. Provide a triage service,actively managing or referring patients or treating patients within own scopeof practice

c. Support the clinical team inthe development of individual treatment plans for chronic disease patients

d. Identify, manage and supportpatients as risk of developing long-term conditions, preventing adverse effectson the patients health

e. Provide routine nursing careto patients as required in accordance with clinical based evidence, NICE

f. Provide wound care (ulceretc.) to patients

g. Request pathology services asnecessary, processing and interpreting results as required

h. Refer patients directly toother services as appropriate

i. Maintain accurate clinicalrecords in conjunction with extant legislation

j. Ensure read codes andtemplates are used effectively

k. Chaperone patients wherenecessary

l. Assist GPs with minor surgerywhen required

m. Prioritise health issues andintervene appropriately

n. Support the team in dealingwith clinical emergencies

o. Recognise, assess and referpatients presenting with mental health needs

p. Recognise, assess and referpatients with community health needs, developing strategies for effective carepathways

q. Implement vaccinationprogrammes for adults and children

r. Liaise with external services/ agencies to ensure the patient is supported appropriately (vulnerablepatients etc.)

s. Delegate clinicalresponsibilities appropriately (ensuring safe practice and the task is withinthe scope of practice of the individual)

t. Support the clinical teamwith all safeguarding matters, in accordance with local and national policies

u. Understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriately

a. If an independent prescriber,prescribe in a safe an effective manner, in accordance with PGD and extantlegislation

c. Monitor and ensure the safestorage, rotation and disposal of medicaments

d. Contribute to theimplementation of an effective training programme for staff and students withinthe practice

e. Provide clinical supervisionfor the nursing team

f. Participate in localinitiatives to enhance service delivery and patient care

g. Support and participate inshared learning within the practice

h. Continually reviewclinical practices, responding to national policies and initiatives whereappropriate

i. Be aware of andcontribute to the planning and implementation of ICB's patient pathwayinitiatives

j. Participate in thereview of significant and near-miss events applying a structured approach root cause analysis (RCA).

k. Maintain professionalregistration and adhere to revalidation requirements
