Digital Transformation Lead at Central Camden Pcn

ROLE AND RESPONSIBILITIES The following are the core responsibilities of the Digital Transformation Lead. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels. Strategic Attain high standards of data quality and support the PCN and Member Practices in the effective use of searches, audits and recalls.
Support the creation of performance dashboards and data reports to ensure targets are met and patient outcomes are improved. Embed quality improvement methodology within PCN Member Practices to support long-term and sustainable change. Support clinicians to undertake quality improvement audits and activities. Monitor key metrics and milestones; and provide timely project/service data to the PCN Executive Team.
Enhance the PCNs digital maturity and drive transformation activities across the PCN through the identification and implementation of digital solutions and technology. Digital Transformation Develop and maintain an in-depth understanding of the PCN digital infrastructure and business intelligence tools. Identify, evaluate, and implement digital technologies, systems, and tools that enable streamlined workflows, data-driven decision-making, and improved patient engagement. Lead and facilitate organisational change, ensuring all stakeholders are aligned with the PCNs digital transformation goals and equipped with the necessary skills to embrace digital solutions effectively.
Support the development and implementation of digital transformation bids. Identify and embed software systems to support the delivery of PCN services. Work closely with GP practices, healthcare professionals, and other key stakeholders to understand their needs, challenges, and opportunities for digital integration. Develop training programs and provide ongoing support to staff members to promote digital literacy and ensure the successful adoption of new technologies.
Improve digital accessibility for patients across the PCN. Leadership and Management Proactively project manage all digital projects and conducts evaluations to determine effectiveness and wider outcomes. Ensure all staff receive training on the digital tools within the PCN. Ensure all digital initiatives are fully scoped and underpinned by a quality improvement methodology.
Remain up to date with the national and local PCN guidance. Stay informed about performance and quality indicators across all facets of primary care. Oversee the management and governance of data, ensuring compliance with data protection regulations, and leveraging data analytics to derive actionable insights. Ensure all Data Protection Agreements, Data Protection Impact Assessments and Privacy notices are kept up to date.
Quality Improvement Work with the QIST Clinical Lead and PCN Quality Improvement Support Team (QIST) on PCN DES/IIF targets. Assist with the creation of data and dashboards in collaboration with the PCN Quality Improvement Support Team (QIST). Construct and run searches on behalf of the PCN and PCN Member Practices using the clinical system, EMIS Web. Prepare and produce reports, spreadsheets, and presentations as required.
Interpret and input data for audit purposes. Support the PCN team with data reconciliation. Perform data quality checks and undertake, or direct, the correction of any errors. Regular monitoring and reporting of key performance indicators for PCN services and PCN Member Practices.
Write data quality related policies and user guides for PCN services. Provide specialist data quality knowledge to PCN Member Practices. Visible presence at practices and meetings to ensure staff training and improvement on systems and data quality. Communication Build effective relationships with the PCN Executive Team, PCN Member Practices, PCN team members and external stakeholders.
Provide clear and regular updates to the PCN Executive Team. Develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice. Deliver technical information in a non-technical and engaging way to a range of internal and external stakeholders. Provide training to staff as required, ensuring the effective use of systems.
External Community Pharmacies, Care Homes, Local Providers (C&I, CNWL, RFL, LBC, UCLH), NCL ICB, NCL Federations, NCL PCNs and GP Practices, NCL STP, NHSE, CQC, PPGs. Confidentiality / Data Protection / Freedom of Information Post holders must maintain the confidentiality of information about patients, staff, and other health service business in accordance with the Data Protection Act 2018. Post holders must not, without prior permission, disclose any information regarding patients or staff. Moreover, the Data Protection Act 2018 also renders an individual liable for prosecution in the event of unauthorised disclosure of information.
