The role of the RTT Outsourcing Coordinator is to work closely with divisional and operational teams to ensure that all data relating to Referral to Treatment Times (RTT) for patients treated offsite are accurately recorded, up to date, and reflect national and local policies. This covers both admitted and non-admitted elective care and is used to support the Trust to manage its critical access targets relating to 18 weeks.

The post holder will be responsible for the efficient management of specific waiting lists from the date patients has been identified for outsouring to independent sector/ mutual aid, to the day whene the transfer is complete in compliance with local and national performance targets and best practice.

The post holder will liaise with local service teams, internal departments and external organisations to ensure continuity of patient care as well as monitoring of internal and national targets, prioritising patients in terms clinical urgency and booking in chronological order.

The role will involve working closely and building relationships with a range of multidisciplinary teams across the entire organisation, but particularly within the Division of Surgery. The post holder must possess excellent communication and interpersonal skills and the ability to work flexibly to enable the smooth delivery of service.

The post holder will be expected to work effectively within the multi-disciplinary team to ensure the provision of a high quality, patient-centred service and develop a clear understanding of all processes to ensure contract delivery.

To be responsible for managing patients RTT pathways, specific waiting lists from the date patients has been identified for outsouring to independent sector / mutual aid, to the day whene the transfer is complete in compliance

To fully involve the multidisciplinary team in the planning of transfers, ensuring that the patient is provided with appropriate information regarding their planned care in a timely manner.

To identify the needs of patients in conjunction with their transfer i.e. language barriers, anxiety, carer / dependency issues.

To manage waiting list activity and admission data with Trust IT system (i.e iCare) and other associate IT systems and action / highlight any errors.

Maintaining thorough and accurate records of all correspondences with patients throughout their admission process, including updating of the iCare system as appropriate.

Update iCare System after agreeing a transfer with the patient to ensure accurate records.
