Pcn Care Coordinator

Modality Partnership Haydock has a full vacancy for a Care Coordinator. The postholder will be a high performing employee who will work as part of a multi-disciplinary team across Modality Haydock Primary Care Network practices. Care coordinators provide extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals.
We are seeking dedicated postholders to join our team and who can display our CARE values: Commitment, Accountability, Respect and Excellence.
This job is suitable for individuals who have a passion for working in health care, making a real difference providing seamless patient care, and who are committed to make real changes to the health care services provided within Primary Care to improve patient care.
The job is not suitable to people who are not resilient to working in a fast-paced environment, working flexibly in line with ever changing demands in Primary Care and the wider NHS.
