Modality Partnership Haydock Division has a full-time vacancy for a Social Prescribing Link Worker. The postholder will work as part of a multi-disciplinary team within general practice, across Modality Haydock Primary Care Network (PCN). Modality PCN supports patients in the Newton and Haydock are from 6 practices across the city. We are seeking dedicated postholders to join our team and who can display our CARE values: Commitment, Accountability, Respect and Excellence.
This job is suitable for individuals who are career driven and have a passion for working in an agile manner, and who are committed to make real changes to the health care services provided within Primary Care to improve patient care. Working with our other PCN roles and general practice colleagues, including health and wellbeing coaches, care coordinators and physician associates, the postholder will provide holistic and person-centred support.
The job is not suitable to people who are not resilient to working in a fast-paced environment, working flexibly in line with ever changing demands in Primary Care and the wider NHS.

**Main duties of the job**
The role of the Social Prescribing Link Worker is to provide social support to patients across the PCN. You will work individually with patients, taking the time to listen and understand what matters to them, and supporting them to access additional services, groups or social activities.
The postholder will need to confidently evidence and utilise key skills such as effective communication, ability to empower people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support.
You will carry and manage a case load of patients, working independently within your remit to support patients. All social prescribers receive clinical supervision.
The postholder will work in collaboration with local divisions (including Divisional Boards) and the National Modality Business Support Team to deliver innovative ways of working to meet the strategic aims of the organisation.
The role is not a regular 9am-5pm job, the job requires innovation, flexibility, and commitment. The postholder will be required to work resourcefully as part of the team to ensure tasks are completed.
