**JOB TITLE: Care Coordinator**

**REPORTS TO: Practice Manager**

**HOURS: 37 hours**

**Job Summary**:
The Care Coordinator will play an important role within the practice team proactively identifying and working with patients, including the frail/elderly and those with long-term conditions to provide coordination and support in the management of their care. The role involves working closely with the practice and multidisciplinary teams within the PCN and is pivotal in ensuring all patients receive the best possible care and service.

The Care Coordinator will support the achievement of the practice’s PCN and Quality and
Outcomes Framework initiatives. To audit and maintain an overview of clinical achievement within the practice and ensure the efficient and timely recall of patients for clinical review ensuring coordination of patient care. The role will include responsibilities for the coordination of the patient’s journey through primary and secondary care.

The applicant will work with the practice teams to enable the achievement of practice targets, national directives, policies and priorities.

The role will include supporting digital initiatives and includes responsibilities for the coordination of the patient’s journey through primary care.

**Responsibilities**:

- To assist the partners and other clinicians in the provision of a high quality service.
- Work with patients, families and carers to improve their understanding of their conditions and support them to develop and review personalised care plans to manage their needs and achieve better healthcare outcomes.
- Help patients to manage their needs through answering queries, making and managing appointments and ensuring they have good quality information to help them make choices about their care.
- To liaise with doctors and other team members to identify patients who have long term health needs and how the practice team can support these.
- To liaise with multi agencies including secondary or social care, patients, carers, care homes supporting the patient with coordination of their care.
- To help patients to understand their health needs in order to help them make quality choices in relation to their care.
- Processing correspondence and associated information to meet clinical quality requirements and targets.
- Reviewing, filtering and flagging information requiring urgent action to clinicians.

- To undertake regular audits and in liaison with doctors implement change and actions to support patient safety as required.
- Running reports on Emis and using the data to identify workload and population groups that require action and support.
- Supporting the partners in achievement of QOF and PII targets and audits.
- Ensuring accurate data recording with appropriate coding.
- Completing and submitting quality alerts as appropriate.
- Dealing with requests for information.
- Initiate contact with, and respond to, requests from patients, other team members and associated healthcare professionals in relation to queries.
- Organising and attendance at MDT and clinical meetings and ensuring relevant actions are implemented.
- To assist with the management of operations.
- Ensure the practice complies with safe working principles at all times in accordance with legislative requirements and practice policies and procedures.
- Help to maintain a positive, supportive patient focussed culture across the whole practice team.
- Participate in training programmes as agreed.
- Participate in peer learning with other Care Coordinators within the PCN.
- To develop competences to enable clinical system data management and reporting.
- Continuously help review, plan, develop and support new systems and working processes within the practice to optimise the quality of monitoring systems.
- Deal with queries from other professionals/organisations.
- Respond to queries via tasks as required.
- Sharing of systems and protocols with the wider PCN team.
