An exciting opportunity has arisen for an experienced RN/AHP to join the Long Covid Service as the Specialist Care Coordinator. The Suffolk & North East Essex Long Covid Assessment Service (SNELCAS), which started on 1 December 2020, has enabled access for patients to system pathways for the full range of post-Covid symptoms supporting GP referral.

Working with colleagues across the Integrated Care System, we have established the SNELCAS clinic, which operates both virtually and face to face, to assess and triage patients with Long Covid symptoms, ensuring they are rapidly signposted or referred directly onwards to the appropriate intervention, service or specialist clinic for their needs. Increasingly the SNELCAS team are delivering self management support and therapy to patients directly, 1:1 and in groups therefore reducing the need for onward referral.

The team includes a single point of referral through the Care Coordination Centre and assessment by a multidisciplinary team of Alliance based Clinicians who form a point of contact for the patient, GPs and various interventions across the West Suffolk, Ipswich & East Suffolk & North East Essex Alliances.

The team is supported by weekly MDT meetings which include a GP and Acute Care Consultant.
- Assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge.
- Work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.
- Be responsible for developing new and innovative clinical practices responding to emerging knowledge and techniques and working with other services where appropriate to make improvements.
- Coordinate the sometimes complex pathway for patients to ensure joined up care.
- Provide skilled therapy clinical expertise with specialised knowledge in the area of management of Long Covid.
- To provide professional leadership involving recruitment, supervision, appraisal and support and development of staff
- To promote high standards of professional practice and the leadership of delivery of quality care within SNELCAS.
- Plan and provide highly specialised training/education programmes for nurses, therapists and other disciplines in relation to Long Covid.
- Maintain a high level of awareness of relevant research trends and national recommendations regarding Long Covid.
- In partnership with the SNELCAS service lead, Finance Advisor and Human Resource Manager ensure budget setting and department finance issues are maximised to effect positive performance targets
- To be responsible and accountable for ensuring the SNELCAS team complies with clinical governance and quality.
