**Key Work Area**:The Advice and Support Co-ordinator’s primary function is to identify and agree with the patient what non-clinical support elements need to be in place to facilitate a smooth and speedy departure from the hospital. The role involves working closely with the multidisciplinary teams (MDTs), Discharge Teams and other organisations supporting patients within the hospital and in partnership with Social Prescribers throughout Suffolk.

The role will support the patient, their family and carers in co-ordinating all key, non-clinical activity that needs to be in place prior to the person being able to be discharged from the hospital. This process will start at the point of entry at the Emergency Assessment Unit with a consultation meeting with the patient. Advice and Support Co-ordinators develop and agree a

Personalised Care and Support Plan for the patient during their journey in hospital to support for their timely exit from hospital. They will ensure the plan is timely, focussed, efficient and person centred.

The role will track the patient through their stay in hospital to ensure any required changes to the care and support plan can be actioned in a timely manner and that the person is kept up to date with such changes.

**Key Responsibilities**

**Service Delivery**
- Support referrals from (primarily) within the Emergency Assessment Unit
- Provide personalised support to individuals, their families and carers to enable a smooth and timely departure from hospital.
- Help people identify the wider issues that impact their health and wellbeing, such as money and debt, poor housing, being unemployed, loneliness and caring responsibilities, benefits and access to statutory and voluntary sector services.
- Develop a co-produced Personalised Care and Support Plan to address patient’s non
- clinical needs following their stay in hospital.
- Develop trusting relationships with patients by giving people time and focus on what matters to them.
- Take a holistic approach, based on the patient’s priorities and the wider social economic issues affecting someone’s ability to leave hospital (e.g. housing, money, benefits etc.)
- Manage and prioritise your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload.
- Ensure patient is connected to their relevant Connect for Health, Community Connector or Social Prescribing Link Worker prior to hospital discharge.
- Work collaboratively with all departments within the hospital to provide a seamless service for the patient.
- Ensure all necessary data and information is recorded accurately and entered confidentially on the relevant IT system with awareness of information governance best practice and Data Protection regulations.

**Administration**
- Advice and Support Co-ordinators may on occasion have a role in educating non-clinical and clinical staff within the multi-disciplinary teams on the service provided.
- To use resources, technology and materials effectively and efficiently to meet wider needs of the patient and other departments within the hospital or outside of the NHS systems.
- Take part in the evaluation of the service through the gathering of data and case studies
- Build relationships with staff in the hospital, attend relevant MDT meetings.
- Contribute information to help the Team identify any unmet needs within the system and gaps in statutory and community provision.
- Use IT for statistical recording, record keeping and document production.
- Ensure all necessary data and information is recorded accurately and entered confidentially on relevant systems with awareness and compliance of information governance best practice and Data Protection legislation
