IDS Early Intervention Team Flow Coordinator
- Band 4 (Permanent)
As part of the service developments within the Early Intervention Team (EIT) and the context of the wider reconfiguration intentions for Integrated Discharge Services (IDS) across BNSSG, the role of a EIT Patient Flow Coordinator has been approved.
The Patient Flow Coordinator role is intended to enhance patient experience, reduce admissions and length of stay, develop robust and efficient discharge processes.

In this vital role, you will
- Maintain momentum of discharge planning throughout the entire hospital process; supporting and working in partnership with other members of the Multi-Disciplinary Team (MDT) to identify patients early and manage expectations of hospital provision.
- Act as a first point of contact for both internal and external enquiries relating to patient discharge as part of the Early Intervention Team model, working with community partners and voluntary services to reduce delays.
- Maintain effective communication with patients, relatives and members of the Multi-disciplinary team regarding all aspects of care demonstrating a variety of communication skills in accordance with the patient group.
- Undertake assessments in order to complete admission and social assessments to commence discharge planning, including all associated documentation.

- The purpose of the role is to assist with the patient pathway from admission to discharge, with early identification of any needs required to support discharge including considering admission avoidance.
- The role is intended to help the Trust reduce length of stay and develop robust and timely discharge processes.
- Attend Board Rounds as required within the EIT.
- Liaise daily with the case manager or team lead.
- Following Board Rounds liaise with the nurse in charge/coordinator and consultant to update on actions required to expedite discharge.
- Post holder will escalate problems in discharge to the case manager or ward team as appropriate.
- Recognise the need for and support or complete referrals to specialist practitioners/therapists as appropriate.
- Participate and support case managers in ward education programmes to develop knowledge and understanding.
- Ensure all diagnostic requests have been made and actioned. Ensure results are returned in a timely manner and are reviewed by the relevant department. Chase individuals and departments as necessary.
- Ensure all patients flow electronic whiteboard and CareFlow is up to date. This must be done in liaison with the nurse in charge / coordinator / IDS case manager. The post holder will ensure patients and carers are aware of their EDD.
- Provide patients with written discharge related information e.g. Trust Discharge leaflet.
- Maintain effective communication with patients, relatives and members of the Multi-disciplinary team regarding all aspects of care demonstrating a variety of communication skills in accordance with the patient group.
- Complete and assist in the completion of Transfer of Care Documents (TOCD)
- Review Transport requests & check bookings made at the earliest opportunity.
- Review patient delay coding with ward staff, case manager & integrated discharge team, assist in updating information.
- Liaise with community partners to update D/C plans and request services on D/C.
- Liaise with care homes/home care providers to ensure early identification of any barriers to timely discharge, implement actions required and ensure staff are aware of any Criteria Lead Discharge (CLD).
- Maintain clear concise patient records and documentation adhering to national standards and trust policies and procedures.
- Provide concise handovers to all members of the Multi-disciplinary team.
- Provide support in collecting data for audit.
- Patient Flow Coordinator will provide a 7 day service.
