This is an exciting opportunity to join the integrated discharge service team and support the momentum of discharge planning from admission to timely, safe and effective discharge.

Through the winter we are looking to increase our expanding Complex Care speciality as well as our established teams across the trust.

This Admin based role involves liaising and co-ordinating with multiple teams across the Trust to ensure patients are discharged from the hospital safely.
- Educated to GCSE standard to include English
- Able to manage conflicting demands
- Knowledge of patient administrative systems - eg Lorenzo/CFC (desirable)
- Previous experience in an clinical environment (desirable)

Full training and support will be given as required.

**In this vital role, you will**:

- Work in partnership with other members of the Integrated Discharge Service (IDS), Ward Multi-Disciplinary Team (MDT) and Community partners, to support decisions with regards to discharge planning
- Represent the Integrated discharge team at ward daily board rounds
- Provide effective & timely communication of discharge actions using existing communication tools.
- Be expected to undertake a range of duties without direct supervision but will be required to report back on those delegated duties to the ward team and case manager.
- Be competent to work across multi
- professional disciplines and make referrals for additional assessments and treatments as required, supported by the case manager.

The purpose of the Discharge Tracker is to assist with the patient pathway from admission to discharge avoiding delays. The role is intended to help the Trust reduce length of stay and develop safe and efficient discharge processes
- Attend the Daily Board Rounds. This is part of the patient review process and will ensure the post holder understands where the patient is in their pathway and patient tracking actions required
- Meet daily with the Case Manager for the allocated area
- Following all ward rounds liaise with the nurse in charge/coordinator and consultant to update on actions required to expedite safe discharge
- Post holder will escalate problems in discharge to the case manager or ward team as appropriate
- Recognise the need for and undertake referrals to specialist practitioners/therapists as appropriate
- Participate & support case managers in ward education programmes to develop knowledge and understanding of complex discharge management including SRF completion and managing patient expectations.
- Ensure all patients care flow connect and flow whiteboard information is up to date and criteria to reside form is completed. This must be done in liaison with the nurse in charge / coordinator / case manager. The post holder will ensure patients and carers are aware of their EDD
- Provide patients with written discharge related information e.g. Trust Discharge leaflet.
- Maintain effective communication with patients, relatives and members of the Multi-disciplinary team regarding all aspects of discharge planning demonstrating a variety of communication skills in accordance with the patient group
- Assist in the completion of the single referral form (SRF) Continuing Health Care (CHC) referrals
- Prompt medical staff to prescribe medicines to take away (TTAs) 24hrs before D/C (48 hours for compliance devices)
- Review Transport requests & check bookings made at the earliest opportunity
