Care Coordinator

Key responsibilities To be the point of contact for service users and their families/carers where information on their short-term care support is required, where they wish to make enquiries about; anticipated visits and or actions of team members or to raise concerns about their short-term care support provided To sign-post referrals to team members and other services as appropriate. Monitors, tracks and reports on progress within the patient pathway. Escalate any issues or delays to relevant Team Lead for appropriate action. To process and administer referrals made to short-term care according to the relevant health criteria seeking advice from relevant team members when required.
To provide a client centred approach to ensure all clients and carers views and opinions are considered and all people who make an enquiry are given respect, dignity and understanding. To request completion of an initial holistic assessment by a relevant MDT team member. Main tasks To process referrals in a timely manner, highlight and resolve any issues or incomplete information with proactive liaison with the referral single point of contact. To work with the MDT members to produce detailed care plans for use in the ongoing provision of care.
To ensure systems are in place ensure appropriate care plan reviews are undertaken by team members. To act as co-ordinator to ensure actions by members of the MDT have been completed in a timely manner. To proactively work with the team to organise short-term packages of care at short notice to prevent unnecessary admission to hospital. To participate in the daily MDT meeting, ensuring that all relevant information is available to team members To contribute to delivering a high quality, efficient and effective service at all times complying with all relevant policies, procedures, guidelines and legislation.
To develop an understanding of community resources/services available within the community to meet client needs e.g. to support initial contacts / sign-posting of information or process referrals. To ensure the information is produced in an appropriate format and made available as required to team members and members of the public To use IT systems, inputting referrals, assessment and care requirements and allocating cases on the Sussex Community Foundation NHS Trust (SCFT) system To collate, input client related (or other required) data and produce relevant data for statistical analysis and to produce reports using IT packages including Excel and Access To record and collate relevant client information on Excel etc as part of ongoing evaluation/assessment/review and to track progress of open cases To ensure all recorded information is accurate, up to date and factual To undertake surveys or audits as required by the team To undertake duties in an autonomous manner within standard operating procedures seeking advice from the team members as appropriate. To contribute and work with MDT colleagues to support the ongoing development and improvement of systems and processes Organisation chart General Manager Clinical Service Manager Operational Team Leader Care Coordinator Health and Therapy Assistant The post holder may have tasks or responsibilities delegated to them, appropriate to their level of competence.
They may also be expected to delegate tasks or responsibilities to other staff, as appropriate. The Delegation Policy sets out the expectation of staff regarding the safe delegation of clinical care and therapeutic interventions to unregistered practitioners and support workers. Flexibility a) This job description is intended to provide a broad outline of the role. The post holder may be required to carry out other duties commensurate with their banding and competence.
Policies and procedures a) The post holder is required to familiarise themselves with all Trust policies and procedures and to comply with these at all times. Confidentiality and data protection a) The post holder must maintain the confidentiality of information about patients, staff and other health service business and meet the requirements of the Data Protection Act (2018) and General Data Protection Regulation (2018) at all times. b) The post holder must comply with all Trust information and data protection policies at all times. The work of a NHS Foundation Trust is of a confidential nature and any information gained by the post holder in their role must not be communicated to other persons except where required in the recognised course of duty.
