About the Role

We are seeking a dedicated and compassionate Care Manager to join our team at Hallandhouse, an independent care home that provides person-centered support to individuals with learning disabilities.

Job Description

As a Care Manager, you will be responsible for providing high-quality care and support to our residents, enabling them to enjoy a fulfilling and valued life. Your duties will include:

  • Coordinating effective care planning and ensuring that staff members have the necessary skills and experience to meet the needs of our residents.
  • Liaising with external professionals, such as Speech and Language Therapists and Occupational Therapists, to ensure that our staff understand and implement guidelines accordingly.
  • Administering medications safely and recording any changes or errors.
  • Maintaining accurate records and reporting incidents and accidents to the Registered Manager/Deputy Manager.
Required Skills and Qualifications

To be successful in this role, you will need:

  • A passion for enhancing the lives of others and a commitment to person-centered care.
  • NVQ Level 3 or equivalent qualification in Health and Social Care.
  • Previous experience in a care management role, preferably in a similar setting.
