Disability Case Manager

Maven Consulting Group Ltd seeks an experienced Disability Case Manager to join their team. The role involves conducting daily assessments of individuals with disabilities or health conditions.
This entails carrying out telephone, video or face-to-face assessments to understand how a person's disability affects their daily life.
You will listen to their experiences, ask questions and use your insights to produce a factual report to help the Department make informed decisions.
As a Disability Case Manager, you will work closely with clients to gain a comprehensive understanding of their needs and provide recommendations for support.
The successful candidate will have excellent communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds.
A typical day in this role may involve:
- Conducting daily assessments to gather information about a client's disability or health condition.
- Collaborating with healthcare professionals to ensure accurate and up-to-date information.
- Producing high-quality reports that meet the Department's standards.
Salary: £35,000 - £40,000 per annum (dependent on experience).
