About the Role
">The NHS Sussex ICB is seeking a skilled Care Coordinator to join their team. As a key member of the organization, you will be responsible for delivering high-quality care services to individuals in need. Your primary focus will be on assessing patients' needs, developing personalized care plans, and ensuring seamless transitions between healthcare settings.
Key Responsibilities:
- Care Planning: Conduct thorough assessments of patients' physical and mental health needs, identifying areas for improvement and developing tailored care plans that address these requirements.
- Case Management: Oversee the implementation of care plans, coordinating with healthcare professionals and service providers to ensure smooth delivery of services.
- Transition Coordination: Facilitate effective transitions between healthcare settings, ensuring continuity of care and minimizing disruptions to patient services.
Requirements:
- Registered Nurse or Allied Health Professional (AHP) qualification
- Experience working in a healthcare environment, preferably in a similar role
- Strong assessment and case management skills
- Excellent communication and interpersonal skills
