About the Role
We are seeking a motivated and experienced Healthcare Professional to join our Continuing Healthcare (CHC) Service as a Clinical Specialist. As a key member of our team, you will play a vital role in delivering high-quality care to patients across Sussex.
Key Responsibilities
- Assess and review patient needs, developing personalized care plans to ensure the best possible outcomes.
- Work closely with patients, families, and healthcare professionals to coordinate care and ensure seamless transitions.
- Contribute to performance improvement initiatives, identifying areas for growth and implementing effective solutions.
- Manage a caseload of patients, prioritizing tasks and meeting deadlines to ensure timely delivery of care.
Requirements
- Experience working in a healthcare environment, with a strong understanding of the CHC framework and related policies.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Proven ability to manage complex caseloads and prioritize tasks to meet deadlines.
- Knowledge of relevant laws, regulations, and standards, including the Mental Capacity Act and Best Interests decision-making.
