Are you an experienced Link Worker or Social Prescribing Link Worker who is looking for a promotion opportunity. If so an exciting opportunity has arisen for 2 Senior Community Mental Health Workers in our Community Mental Health Support Service.

The Community Mental Health Support Service provides support to people with mental health problems to access mainstream activities and resources and explores issues faced by service users that are referred, including management of their mental health, emotional management, domestic needs, and wellbeing.

You will assist individuals to reduce reliance on or move on successfully from specialist mental health provision, promoting independence by supporting individuals to develop employability and life skills and actively engage them to participate in group activities moving whole groups to independence.

As Senior Community Mental Health Worker, you will working closely with the Team Lead, using your expertise and strong interpersonal skills to help develop and deliver this service. You will be responsible for your own case load and provide support and supervision to other Community Mental Health Workers in the team.

**About you**

We would love to hear from you if you possess the following:

- Proven track record as an effective link worker / social prescriber.
- Experience of supporting the management and coaching of a team.
- Experience of delivering services in a community-based setting and working independently.
- Are able to work to deliver required performance targets.
- Strong personal skills and evidence of developing positive working relationships with individuals, colleagues and partnership organisations.
- Ability to work effectively as part of a team.
- Good administrative skills with the ability to support the Team Lead with the management of the service.
