**What You'll Do**:

- Uphold the highest standards of care and support, tailoring to individual needs with a person-centred approach.
- Conduct assessments, plan and implement suitable care strategies in collaboration with residents, their families, and our care team.
- Develop and regularly update personalized care plans and risk assessments.
- Foster positive relationships with residents and their families, ensuring care delivery respects each individual's dignity, mobility, and personal choices.
- Monitor and respond promptly to changes in residents' conditions, reporting significant concerns to the management team for swift action.

**What We Offer**:

- Comprehensive training, coaching, and support to fully equip you for your role.
- A competitive salary of £11.55 per hour, rising to £12.60 per hour in April 2024, with annual reviews.

**Requirements**:

- Proficiency in MS Word and Outlook, alongside strong communication and organizational skills.
- A minimum of 2 years' experience in Health and Social Care, including at least 1 year at a Senior Level or equivalent.
- NVQ Level 2 in Health and Social Care is a must, with a preference for those working towards Level 3 (or equivalent).

**Additional Details**:
This role involves a rotational shift pattern, covering evenings and weekends.
