Interim Advanced Social Work Practitioner - Children’s Mental Health
24 per hour PAYE
Hours per week: 37 Monday – Friday, 08:45 - 17:00
Children’s Advanced Social Work Practitioner to join a new and innovative team specialising in children’s mental health. Support children and young people who have complex mental health issues, both at home, in placements and within specialist mental health hospitals.
Ensure that there is a robust safety and care plan in place, that keeps the child/young person at the centre. Develop care plans in collaboration with the family and specialist agencies involved in the case, such as CAMHS, NHS England, Clinical Commissioning Group and specialist hospitals.
Manages a workload of the most complex cases which includes looked after children, child protection and assessment of carers.
Carry out assessment and investigations into cases involving significant harm, child protection or assessment of carers and compile reports to case conferences, courts/statutory panels in accordance with statutory procedures and best practice.
Implement, monitor and review plans relating to the assessed need of children and young people, their families and carers.
Produce assessments of need for children and young people in line with statutory regulations and ensure that services are available to meet those needs.
Undertake statutory supervision and care planning in relation to children who are looked after.
Act as a Practice Teacher to students on placement.
Maintain performance and develop practice and experience in line with personal targets as agreed with the Senior Fieldwork Manager through supervision.
Contribute to the achievement of Key Performance Indicators identified through service planning.
Work to the prescribed quality standards for service provision set out by the service for all workers.
