Leadership Provide a visible, accessible and professional team leadership presence for all community team staff, service users and their families across community services. Act as professional lead for all our community services. Have overall responsibility for the safe and effective delivery of adult diabetes services. Ensuring high standards of care and safe practices are delivered in line with national guidance and best practice is fully is embedded.

This includes ensuring best practice arising from national reviews is fully embedded in local practice. Represent our community services at local system and ICS meetings/events relevant to the delivery of community diabetes services where required. Build a strong alliance with our system wide partners to ensure representation and service development can be maximised. High Quality Clinical Service Delivery Ensure diabetes care is person focused and underpinned by evidence based practice and continual knowledge and skills of staff.

Ensure that patients privacy and dignity are protected and that patients are treated with respect at all stages of their patient journey. Support systems for measuring and improving the quality of the patient/care experience for adult patients with diabetes. In collaboration with the service manager, ensure there are systems in place to learn from all patient/carer experience safety. Ensure delivery of core contractual Key Performance Indicators (KPIs) and targets / expectations.

To review outcomes information and develop systems of audit to ensure continuous improvement. Team Management Ensure effective skill mix for best possible clinical care and provide safe, effective levels of care. Provide strong leadership and management to the team with support, supervision, appraisal and professional development plans. Ensure all direct reports have clear team goals and strategic guidance for the overall service.

Encourage professional development within the team and work closely to develop strong leadership from direct reports and other staff. Implement systems and controls for the effective recruitment, selection and retention of nursing staff for the Adult Diabetes Service. Support all direct reports to plan for 100% compliance with statutory and mandatory training for nursing staff on an annual basis. Support all direct reports to plan for 100% compliance with appraisals and personal development plans for all clinical staff on an annual basis.

Team Development Oversee the production of team development plans that ensure that all staff aligned to Adult Diabetes Service achieve the required competencies for their role and have the opportunity to access additional training and development in line with their appraisal personal development plans. Support the implementation of extended clinical practice in diabetes to modernise the service to benefit patients and their families care. Support the progression of the Adult Diabetes service and ensure relevant redesign is supported to benefit patients and their families In collaboration with senior clinical colleagues and the system develop and review workforce planning including annual reviews of skill mix to ensure a high performing, safe and effective service. Ensure the Adult Diabetes team meet their responsibilities to provide clinical experience and mentor support for pre and post registration nurse education, in line with internal and external standards.

Ensure a system is in place to analyse the training and development needs for Adult Diabetes Team nursing, clinical and support staff. Support the service to become a high performing and clinically recognised service of excellence. Support the development of integrated pathways across community and secondary care services. Develop effective working relationships, ensuring seamless delivery of care with community nursing, intermediate care, mental health and other specialist teams.

Develop the team to support the implementation of new technology. Governance Undertake management and investigation of incidents and SIs in relation to the Adult Diabetes Service. Where required manage complaints relating to the Adult Diabetes Service in line with agreed Trust processes and where required to resolve complex nursing complaints, meet with complainants to address their concerns to ensure local resolution is achieved wherever possible. Ensure quality and implement effective systems throughout the service including liaising with other agencies around, Safeguarding, human rights and best interest decisions.

In collaboration with the senior management team, support the Directorates approach to improving compliance with NHSLA standards, CQC registration and other regulatory requirements. Work with the senior management team to ensure the delivery of the Directorate and Corporate objectives. Support the Directorate with the delivery of the CQUIN and QIPP agendas as required.
