The personalised care pathway tracker will provide support to the Cancer Services team by proactively tracking patients from referral, through diagnostics, treatment planning and all screening, upgrades and subsequent treatments in accordance with national guidance. Work with the gynaecology clinical nurse specialists to support patients undergoing a gynaecological cancer diagnostic pathway, patients with a cancer diagnosis or patients living with and beyond cancer. Work closely with clinicians, specialist nurses, admin staff and all members of the Multi-Disciplinary Team to ensure that patients are actively managed through the cancer pathway. Tracking patients through the delivery of personalised care following a cancer diagnosis. Support and escalate recurrence of cancer to the nursing team. Support ongoing wellbeing programmes

To work closely with clinicians, specialist nurses, secretaries and all members of the Multi-Disciplinary Team (MDT) across all specialties to ensure that diagnostic tests have been requested and work with the Cancer Nurse specialists to manage coordination of results and appointments. To ensure that appropriate requests for investigations are managed in a timely fashion. If diagnostic delays occur or appear imminent it is the responsibility of the post holder to escalate to the relevant manager. Coordinate the follow up investigations required post cancer diagnosis or treatment and provide a point of contact for patients post cancer treatment. Liaise with all diagnostic departments internally and externally to ensure that patient’s tests are booked, undertaken and reported.

To be fully conversant with patient pathways across site specific tumour groups to ensure that patients are treated equitably, reaching targets for holistic needs assessments and diagnostic tests. To liaise with MDT coordinators and across the Trust to monitor the coordination of all elements of the patient pathway. To provide support to the team to ensure continuous development, implementation and monitoring of changes to the patient pathway. To be conversant with all reports available on the Somerset database to manage the patient pathway. Responsible for coordinating rapid re-entry for specialist review against a clinically approved triage model. Development of survivorship and wellbeing services. Support the delivery of the Recovery Package in line with the Cancer Alliance. Support and participate in audit processes and service development. To identify delays in the patient pathway and to follow the trust escalation policy to raise awareness to the Team Lead for Macmillan and Lead Cancer Clinician as appropriate. Working in a team environment to provide appropriate support/advice to other secretarial/clerical staff as necessary. Build relationships with a wide range of staff both internally and externally to undertake job role. Ability to promote patient empowerment and self-management strategies. Deliver self-management advice, support and know when to escalate to clinical team.
