Integrated Frailty Lead

This is a system wide appointment hosted by LCHS. In performing this system wide role, the post holder is required to develop a shared system view of frailty, and to hold stakeholders to account, driving forward the actions that need to be taken as a system, to address shared challenges and implement new ways of working.

Through responsive leadership the post holder will act as a strong patient advocate to ensure that a culture of continuous improvement and development is support in order to drive quality care delivery for frail older adults across Lincolnshire.

The post holder will be expected to develop services through the implementation of innovative strategies to ensure an empowered workforce at all levels that is fit for purpose and equipped to deliver excellence in care standards for frailty, including securing and developing the workforce of frailty practitioners for the future.

The post holder will be proactive, able to make recommendations, facilitates and contributes to the management of change to ensure services are patient and carer focused, evidence based and provide high standards of clinical care; delivering service provision in line with national and local policy.

This is a strategic role and will support the senior leadership structures across the system through effective collaboration as well as ownership of transformation projects and provide a strong and visible professional clinical leadership for the Frailty workforce.
- Work within Frailty work streams and across systems to establish and develop essential processes and governance structures.
- Create and embed new care pathways within Frailty utilising the virtual ward alongside clinical teams as appropriate.
- Create and contribute to effective project management within Frailty and the FVW, for example, ensuring key milestones are achieved.
- Ensure all agreed targets are achieved and a comprehensive set of data is produced monthly to determine the priorities for the services and monitoring required.
