Therapy Advanced Practitioner – Front Door and Frailty (NHS AfC: Band 8a) - Therapy - United Lincolnshire Hospitals NHS Trust

United Lincolnshire Hospitals NHS Trust is seeking a highly skilled Therapy Advanced Practitioner to lead our Front Door and Frailty team. As a key member of our Therapy and Rehabilitation Clinical Business Unit, you will be responsible for providing high-quality care to patients in emergency departments and frailty services.

The ideal candidate will have a strong background in physiotherapy and experience in leading teams. You will be expected to demonstrate a high degree of autonomy and complex decision-making in emergency care and frailty. Your expertise will be essential in providing clinical leadership for the therapy team and frailty team working at the front door.

We are committed to delivering services that are fit for the future and reducing hospital admissions. As a Therapy Advanced Practitioner, you will play a crucial role in achieving this goal. You will work collaboratively with stakeholders across the hospital and community to ensure the delivery of clinical KPIs.

Key Responsibilities:

  • Lead the Front Door and Frailty team

Requirements:

  • Registered therapy professional
  • MSc in a relevant field or equivalent
  • Experience in leading teams and providing clinical leadership
