Job Title: Advanced Clinical Practitioner

We are seeking an experienced Advanced Clinical Practitioner to join our team at Lincolnshire Community Health Service NHS Trust. As an Advanced Clinical Practitioner, you will be responsible for providing high-quality patient care in an urgent care setting.

About the Role

The successful candidate will have a strong background in urgent care and emergency medicine, with experience in assessing, diagnosing, and treating patients with complex undifferentiated conditions. You will be an independent prescriber, able to prescribe and review medication as needed. In addition, you will have excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.

Key Responsibilities
  • Assess, diagnose, and treat patients with complex undifferentiated conditions
  • Prescribe and review medication as needed
  • Work effectively with patients, carers, and other healthcare professionals
  • Develop and implement care plans to meet the needs of patients
  • Contribute to the development of local guidelines and protocols
Person Specification
  • Recognised Advanced Clinical Practice Qualifications at Masters Level
  • Proven and Current Experience in Urgent care/ Emergency Medicine Setting
  • Evidence of Recent Professional Development in the past year and demonstration of seeking & undertaking autonomous learning & Development Opportunities
  • Ability to identify Training need sin other Health Care professionals and formulate detailed teaching plans.
  • Interpreting & Implementing National and Local policy
