- As a Service Coordinator you will join an experienced and supportive team of Social Workers, Nurses Occupational Therapists and Social Care Advisors delivering a positive approach to assessment, safeguarding and crisis management. Gateway to Care and Community Social Work Practice are dedicated to ensure that human rights of individuals are at the forefront of service delivery and adults remain in control of their lives and are supported to make choices that enable their independence wherever possible.- We are looking for a hard-working individual who has experience in doing assessments, supporting individuals to self-help and promote independence who is confident to have skilled strength based conversations.- Service Coordinators work independently and as part of a team. You would ensure that individuals remain at the heart of your work and empower vulnerable adults to make choices and to maintain their own autonomy wherever possible.- You would make sure that the needs of family members, carers, groups and providers of care are understood and will work with them towards ensuring that vulnerable adults receive the appropriate support (which is the least restrictive option) whilst recognising the need for intervention at times.- You would ensure that vulnerable and/or disadvantaged people are supported appropriately; you will make decisions that are in accordance with relevant policy guidance. You will deploy the correct use of legislation aimed at maintaining the independence of vulnerable adults and assist them to maximizing their own abilities and networks of support.**You will**:

- Support people and be responsible for assessing their needs.
- Ensure that the needs of family members, carers, groups, and providers of care are understood and work with them to ensure appropriate support is given.
- Implement and review support plans and provide short term intervention.
- Take an active role in safeguarding investigations with support of professionally qualified staff.
- Liaison with health colleagues, other professionals, and organisations to promote health, wellbeing and avoid hospital admissions where possible.
- Ensure computerised records are accurate, up-to-date, and reflective of the department’s involvement with individual service users.
