**Your role**:
We are excited to be recruiting for 1 WTE Primary Care Network (PCN) Care Coordinators. As a Care Coordinator you will play a crucial role in supporting individuals to improve their health and wellbeing.

You will do this through supporting individuals and families to get access to the right support, and to feel active and confident in managing their own health and wellbeing. You will work as part of the multi-disciplinary PCN team to proactively identify and work with people to provide coordination and navigation of care and support across a range of health and care service. As well as promoting and embedding the personalised care approach across the PCN.

You will act as a central point of contact to ensure support is available to individuals and carers so that they are supported to understand and manage their condition(s). You will work with individuals to develop an individualised care and support plan based on what matters most to them.

If you are passionate for helping people and would love the exciting challenge of providing client focused service within local Primary Care Networks across Wigan Borough - then this could be the job for you.

**About you**:
You will be an innovative, confident, and enthusiastic person committed to achieving positive change for the residents of Wigan Borough. To do this you will have excellent communication, listening and motivational skills allowing you to inspire and produce positive change in people’s lives.

You will have an understanding of the factors that can impact on an individual’s health and wellbeing; as well as a good knowledge of local communities and of the resources and assets that are available within the community to support individuals. You will be an excellent communicator and be able to engage and build connections with other stakeholders.

You will have excellent co-ordination skills and provide timely and efficient support. You have empathy, be able to build on people’s strengths and a passion to improve the lives of people in our borough, by enabling people have more control of their lives, develop their skills to help them manage their health and wellbeing.

This is an exciting new role, so we are looking for individuals that are excited and committed to helping shape these roles with the PCNs.
