JOB PURPOSE The Lead Professional will be the clinical expert and professional resource for the management of patients with Complex Needs. The post holder will support the multi-professional team, patients, and carers, when patients with Complex Needs access secondary care. The Complex Needs service covers (but is not limited to) Learning Disabilities, Dementia, Autism, Mental Health, and Delirium. The post holder will support the multi-professional team, patients, and carers, when patients with Complex Needs access secondary care. The Complex Needs service covers (but is not limited to) Learning Disabilities, Dementia, Autism, Mental Health, and Delirium. The post holder will develop the specialist area of practice in partnership with the Head of Complex Needs and will work collaboratively to develop services and to improve the quality of care delivered. The post holder will manage other members of the team and in addition lead the development and delivery of a programme of practice and professional development for staff within the speciality.

This will be achieved through working in partnership with lead clinicians, managers, and matrons. Maintain a professional appearance in line with the Trust dress code. Maintain professional and courteous working relationships with staff and respect the equality and diversity of each and every person he/she comes into contact with in the course of his/her business. Uphold the privacy and dignity of the patient and respect the equality of patients at all times. Develop a high level of clinical decision making, monitor and improve standards of care, working within their advanced scope of professional practice. Contribute towards a multi-professional approach in the management of patients within the Complex Needs team, especially in relation to ongoing care needs and discharge arrangements.

Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead, or participate in their care delivery. To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management. Support the delivery of adjusted patient pathways for patients accessing secondary care. Provide advice and support on the mental health act to ward staff and support the receipt and scrutiny of Mental Health Act Paperwork.

Be an innovative, enthusiastic role model providing leadership, guidance, and advice to staff on operational and professional issues promoting an open and honest and transparent culture. Provide professional visible leadership and a supportive approach that contributes to creating a climate for achieving excellence in clinical practice, skills and competencies using the values of the Trust. In partnership with the Head of Complex Needs, lead the implementation of service improvements for people with Complex Needs and the evaluation/monitoring of care through audit, complaints monitoring, and incident monitoring and patient/carer feedback. In partnership with the Head of Complex Needs, lead the Trusts Complex Needs Delivery Group providing clinical advice, expert knowledge, and practical support to the implementation of the Complex Needs Forward Plan.

Identify, develop, and sustain mechanisms to support patient involvement and feedback. Be a resource for health care professionals by being visible on the wards, available, and accessible for support and advice in relation to the management of patients in the speciality. Provide leadership to the team and working with the head of complex needs, develop new roles / ways of working (High Impact Changes) linked to directorate business plans. Lead on consultations and develop strategies for delivering effective care within a changing environment that improves the quality of care and health outcomes in the Complex Needs team.

Employ effective decision-making skills to address complex issues and use effective change management skills to implement these. Take the lead in investigation of incidents / complaints when required, including detailed reports / letters to complainants. Work in partnership with the Head of Complex Needs to manage the Complex Needs team towards achieving directorate, and Trust objectives. Identify and develop proposals and business cases as required for future service innovation.
