**Job overview**:
This is an exciting opportunity for a Care Co-ordinator to work with the Ellesmere Port East Care Community Team. You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service.

You will work alongside team and partners including GPs and the voluntary sector, to support patients within the care community.

**Main duties of the job**:
Good interpersonal and communication skills.

Co-ordination of multi disciplinary meetings.

Management of of daily meetings.

Liaising with multi-disciplinary team members.

Signposting team members and patients/carers to relevant services.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
