This is an exciting opportunity for a Care Co-ordinator to work with the Ellesmere Port West Care Community Team.

You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service. You will work alongside team and partners including GPs and the voluntary sector, to support patients within the care community.

This post is part time 15 hours per week, Monday and Wednesday 9-5pm.

This role will include the following
- Good interpersonal and communication skills.
- Co-ordination of multi disciplinary meetings.
- Management of of daily meetings.
- Liaising with multi-disciplinary team members.
- Signposting team members and patients/carers to relevant services.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.
