**The specific tasks a Care Co-Ordinator will be expected to carry out are as follows**:

- Show a good level of communication skills ensuring we are responsive to our client’s needs. Supporting staff, clients, their families, professionals, and our associates to the best of your ability.
- Assessing new clients in a caring manner in their homes, carrying out full risk assessments to ensure the service is safe and effective.
- Ensuring all staff are supervised regularly while working in client’s homes (any concerns must be reported to the manager and a record kept in carers file). This is to support staff and clients alike.
- Produce staff rotas and ensure all staff are given their programme on time.
- Auditing of documentation (staff and client files) and ensuring client and carer compliance.
- Assist managers with reassessment of client’s care needs every 6 months or when required, encouraging and empowering clients to be more independent or try new things.
- Attend any meetings scheduled in the absence of the manager and ensure all contents are communicated effectively, being an inspiration to the team and leading by example.
- Have “on call” phone as agreed.
- Ensure any staff concerns or problems are dealt with effectively and reported to managers, showing compassion when needed. Encouraging openness and honesty as detailed in the Duty of Candour.
- Keep all information about clients and their families secure and confidential in accordance with GDPR regulations.
- Attend all training arranged by management in a timely manner, making sure that every action you take is effective for the client.
