To engage service users in physical and recreational activity appropriate to their individual needs and abilities To plan and co-ordinate an innovative and adaptive programme of activities under the supervision and direction of members of the multi-disciplinary team Organise and plan service user events in collaboration with MDT colleagues to promote inclusivity. To develop and use practical and technical skills with service users, either through your own identification of need or as suggested by members of the multi-disciplinary team To work with service users to improve their mental health, physical health and wellbeing. To assist in identifying service users strengths and needs in order to plan and implement suitable activities for groups and individual service users. To be involved in evaluating the effectiveness of activity with service users, carers and the multidisciplinary team To record clinical information on service users progress with members of the multi-disciplinary team **Please note this role is pending evaluation
