**Care Coordinator & Administrator - 20 hours per week over 5 days (Mon-Fri)**

We are an established Primary Care Network of 3 GP practices in and around Aylesbury; together, our whole team aim to improve the health of our patients, alongside enhancing their experience of patient care with a great family team ethos.

We are educationally focused and offer ongoing training and development whilst providing a supportive, working environment and actively support team wellbeing.

**The Role**

We have over 400 patients in care/nursing homes, so an exciting opportunity has arisen for a Care Coordinator and Administrator to join our friendly multi-disciplinary team (MDT). The successful applicant will work across our Primary Care network, providing a high level of admin support to improve the health and wellbeing of our care home residents

The role will ensure the smooth running of the MDT by providing administrative support for regular care home rounds, developing and maintaining administrative systems, arranging MDT meetings and taking minutes, supporting completion of referrals and liaising with service providers and clinicians to identify the needs of the patient.

This post may involve working across any of our three sites as and when required. You may also be required to travel to any of our care homes to attend meetings.
