Ophthalmology Ivt Pathway Coordinator

JOB SUMMARY To undertake responsibility for the operational co-ordination of the Intravitreal Service and for providing the central point for communication for bookings, appointments and clerical issues within the Eye Clinic. To monitor compliance with clinical standards and NICE guidance for treatment of age-related macular degeneration (AMD), diabetic macular oedema (DMO), central and branch retinal vein occlusions (CRVO and BRVO) and other related retinal conditions. To escalate capacity issues and schedule additional capacity when required ensuring that treatment is carried out within agreed time frames. To work closely with the clinical lead, service manager and matron to ensure that the service is patient focused, efficient and delivered according to national guidelines.
To undertake responsibility for accurate data entry onto the Trust and national databases including prompt completion of BlueTec forms for notification to the local CCG as well as correlation of data returns to meet required time scales. Maintain key relationships with colleagues (see below) to ensure a patient focused and high quality experience in the Intravitreal Service. KEY RESPONSIBILITIES Relationships You will be accountable to the Clinical Director and General Manager for Ophthalmology. You will be expected to work closely with the Service Manager and Rota Coordinator for Ophthalmology Services.
**In addition you will be expected to work directly with**: Clinical Lead for Intravitreal Service Medical Retina Service team Matron and senior nursing team Ophthalmology administrative team Pharmacy Department Information Department IT Department Clinical Management and Bookings Maintain an electronic record / database of patients undergoing treatment and monitoring intervals for treatment on a weekly basis and escalating issues to the Service Manager for Ophthalmology working in close liaison to manage capacity and patient waiting times providing a forecast of demand. Ensure that clinics are planned and prepared at least a week in advance matching capacity to available resources through liaison with nursing, clinical and clerical staff. Liaise with patients to ensure that all clinics are booked appropriately to avoid DNA and cancellation with a choice of attendance dates offered in line with the Trust policy. Ensure new referrals or any other urgent referrals thus received, are booked into the Rapid Access MD1 clinic within 2 weeks of receipt of referral for wet AMD and within 4 to 6 weeks of a retinal vascular disorder such as CRVO / BRVO.
Ensure all patients booked into the MD1 Rapid Access clinic have Fundus Fluorescein Angiograms arranged on the same day requiring liaison with the Ophthalmic Imaging Technician / Photographer. Ensure that patients are booked for their first three or four loading doses of injections where appropriate by liaising with the Clinical Lead for Intravitreal Service. Ensure follow up in the agreed time frames within the one-stop clinics. Ensure availability at reception during the Rapid Access first and one-stop follow up clinics to facilitate appointment bookings and address any queries the patents may have.
Ensure that all staff in the department adhere to standard operating procedures with audit compliance on request. Reporting and Activity Monitoring Ensure that all activity and data requirements for the service are recorded on the appropriate database / Trust IT systems. Ensure that patient attendances and supporting information is entered onto the database and readily available for audit. Support the Medical Retina Service with service improvement and audit.
Provide updates on activity and forecast of future demand as and when required and participate in monthly department business meetings. Coordinate additional capacity when required by liaising with the Service Manager and rota coordinator. Work closely with Clinical Lead for Intravitreal Service and Service Manager to ensure that treatment intervals comply with NICE guidance taking actions to do so where breaches exist. Be responsible on a daily basis for entering all information for each patient onto the appropriate database as well as completion of the notification forms for the CCG; for updating the databases when patients are discharged and liaising with other hospitals where necessary to do so.
Additional Duties To ensure all clerical equipment and stationary supplies are suitably maintained and any faults reported promptly. To ensure an adequate supply of IVT clinic documents in the relevant areas in the Eye Clinic and IVT Injection room. Book lunch meetings with medical representatives To ensure IVT Clinic Sheets / IVT Booking Forms / GP letters etc are filed appropriately and copies posted to the GPs and patients. Maintaining appropriate filing of clinical records of patients complying with the Trusts Information Governance and security policy.
