General duties The Failsafe Officer will ensure that all patients who require a follow up appointment are seen in line with the recommendations of the clinical team. Where this is not possible due to capacity issues, this will be discussed with the clinician and management team to find solutions. The post holder will work in close liaison with administrative and clinical colleagues to ensure that appropriate appointments are booked in a timely manner and provide training to staff as appropriate. The post holder will work very closely with the 18 week team and Outpatient department to ensure the whole patient pathway is maintained and reduce the risk of lost to follow up.

On a day-to-day basis, oversee the functioning of the Ophthalmology Administration Team and ensure that all protocols and procedures are followed. Keep up to date on changes to policies, procedures and recommendations and assist the Assistant Service Manager in adapting existing practices or implementing new requirements. Report all risk incidents appropriately as per Trust policy and Investigate reported incidents on the system and implement any arising actions or lessons to mitigate recurrence. Act as a Super User of the Ophthalmic Electronic Patient Record software, providing the first point of contact for all issues relating to the software system in use and to resolve all issues found in the software as well as testing updates.

Work collaboratively with the Service Manager to ensure that all national and Trust targets are consistently achieved. In conjunction with the Assistant Service Manager, identify the need for extra clinical activity and take a lead on the scheduling and co-ordination of it. This includes securing required supporting resources, such as administrative cover. Work with the clinicians and management teams to monitor, suggest and implement improvements to clinical information flows between primary and secondary care colleagues.

Represent the Service at meetings as required and deputise for the Assistant Service Manager as necessary. To undertake any other reasonable tasks or projects as directed by the management team to support the Ophthalmology service. Data and reporting tasks Provide a full reporting function for the Service. This includes the production of monitoring reports for the Service Line Meeting and Clinical Governance Meeting.

This should include a summary of the Ophthalmology waiting lists and wait times for appointment. Support with the compilation of quarterly reports for the service. Undertake administrative audits and support clinical audits as required. Undertake complex analysis and interpretation of clinical data.

Create and monitor complex data spreadsheets as required. Develop and produce appropriate statistical reports for different audiences, including interpretation of statistical information. Utilise Trust systems such as IPM, Evolve and Medisight to undertake clinical audit and monitor activity. Act as Systems Administrator for Medisight, the Ophthalmic Electronic Patient Record system, responsible for data integrity, completeness, validation, systems access and provide training.

Escalate system faults, errors or problems in a timely and appropriate way. Maintain data protection and information governance at all times, in line with the Trust requirements. Communication and working relationships Work closely with the administrative team within the department, to track patients along their care pathway and ascertain clinical outcomes. This will require the use of negotiation; planning and persuasion skills to ensure that pathway timings are adhered to and that outcomes are recorded consistently and correctly.
