ABOUT THE ROLEWe're hiring a Support Coordinator to join us on a 12-month fixed term contract. You will specialise in supporting family and carers, developing partnerships and resources as the lead for Luton to enable family and carers voices to be heard. You will establish links between established care networks across Luton and offer specialised advice, signposts, advocacy and support for those in an official or unofficial caring capacity. You will contribute to developing trusting relationships with our participants, engaging with them through key working activities, identifying the needs, goals, and skills of the participant and provide them with pathways, support, and appropriate interventions that will enable successful resettlements.

ABOUT THE SERVICE The PenroseMental Health Floating Support(MHFS) service delivers support to individuals living within the community who have support needs pertaining to their mental health and wellbeing. The service works alongside statutory services, with the aim of moving people on to independent living within two years. Who is the service for & what is it funded/ accessed to deliver? Men and women, aged 18+ Our clients have a primary mental health need, including complex needs and dual diagnosis All our clients have a personalised support plans and proactive support work, to engage them with the opportunities available to them to live independently in the community, develop their social skills and independent living skills.

We work with clients to improve their health and wellbeing, including supporting people to manage their mental health condition and crisis prevention. OVERVIEW OF KEY RESPONSIBILITIES Be the point of contact for advice and guidance for the Synergy team and enable a portfolio of resources to be produced, and case studies on additional gaps and areas of needs to effectively and successfully meet the needs of family and carers across the Luton. Take ownership of being the family and carers lead, developing key partnerships and networks with other providers such asBLMK Community Transformation Agenda, the Blended Work with ELFT, The Complex Emotional Needs Transformation Agenda, Healthwatch Luton, the Recovery College and The Luton MH Collaborative. Manage and run group working activities and focus sessions with relevant participants, as well and internal and external partners. Develop strategies for engaging with local schools, to ensure that the SIG-Penrose offer is well promoted, and that Family Workers and Pastoral Care leaders have a point of contact for the families involved with their educational establishments.
