**Responsible to**:Resettlement Team Leader (Luton)

**Jo**b Purpose**:To be a part of a small, proactive resettlement team, providing intensive floating support to tenants aged 18-65 in our outreach supported accommodation properties in Luton. Working with tenants who frequently have a history of entrenched rough sleeping, offending, substance misuse and poor mental health, you will work in a trauma informed way to build effective relationships, tailoring support to the individual and focusing on goals and aspirations rather than needs and issues, to enable residents to progress to sustainable independence.

**Ma**i**n Duties and Responsibilities: *
- To work collaboratively with the LBC Partnership Resettlement Coordinator and other stakeholders to identify and place eligible tenants through the rough sleeper pathway
- As part of the Rough Sleeper Accommodation Pathway (RSAP) funding initiative, you will work with people who are either rough sleeping or have a history of rough sleeping and in emergency accommodation at the point of referral.
- At move-in, support with setting up utilities, budgeting, maintaining neighbourly relations etc. Help tenants to personalise the property (soft furnishings etc.) to encourage ownership.
- Provide intensive, wrap around floating support to those who have experienced rough sleeping to help sustain tenancies in our outreach properties.
- Work with the Housing Officer to ensure rent accounts are up to date and supporting tenants to budget effectively and prioritise rent in their spending.
- Help to develop life-skills that will optimise chances of success in tenancy sustainment.
- Support tenants (at their own pace) to improve health outcomes by engaging with specialist services and develop pathways with community groups to promote a sense of belonging.
- Identify when tenants are ready to move on to independent or stage-2 accommodation and support them with the transition. Provide up to 12 weeks’ follow-on support.
- Ensure that the voices of service users are consistently sought to enable them to shape service delivery
- Perform appropriate health & safety checks, risk assessments, safety plans and maintain accurate records
- Be part of an out of hours’ service on a rota basis with the team to support the residents
- To adhere to and operate within the rent arrears and voids policy and procedures minimising voids and maximising income, where applicable

**Additional Duties and Responsibilities**:

- Communicate effectively with external professionals working with tenants, attending meetings where necessary
- Be instrumental in promoting excellent safeguarding practices in conjunction with Local Authority and IMPAKT Housing & Support safeguarding policies
- Ensure full understanding and operation of In-Form Hostel Management System and ensure client details are updated as appropriate.
- Participate fully as a member of the team to ensure that services are provided and maintained to a high standard.
- Be flexible to the changing needs of the organisation and its service users.
- Abide by the organisations staff working policies and procedures.
- Promote the aims, purposes, mission and services delivered by the organisation.
