Clinical Commissioning Team Lead

The post holder will support the Trust and ICS’s commitment to deliver the ambition of the national learning disabilities and autism programme (previously referred to as Transforming Care (TCP)) through ensuring people, adults and children, are supported in the most appropriate setting to meet their needs. The role will have a focus on supporting people to discharge from specialist hospital placements into the community, or into the least restrictive environment, therefore reducing the number of people who have a learning disability and/or autism who are in specialist hospitals.
To achieve this the post holder will be the lead the clinical commissioning team to commission highly personalised care and support packages to enable people to live meaningful lives in the community in settings which are sustainable, high quality and safe, living as part of the community to allow people to thrive and live their best life. The post-holder will be responsible for leading either the discharge or community hub team, providing leadership and day to day management of the team, ensuring delivery of the team function to enable the Trust to achieve the learning disability and autism programme ambitions. The post holder will build and maintain strong relationships with system colleague to support overall delivery of the programme, and to provide appropriate clinical challenge to ensure people are supported within the most appropriate settings to meet their needs.
Establishment and adherence to locally defined processes and policies based on national guidance for the programme.
Provide clinical leadership and support for all admission avoidance and discharge processes defined within the SOP are enacted to ensure only clinically appropriate admissions take place.
The post holder will provide clinical challenge, where appropriate, to ensure the system takes all appropriate action to support people in the community.
Ensure functions are delivered on a day to day basis through line management of the team, including planning and cover of leave, supervision responsibilities of the team.
Ensure that Mental Health Act, Mental Capacity Act and Deprivation of Liberty Safeguards principles are applied to all cases.
Oversee root cause analysis investigations for admissions/readmissions as required.
Through leadership of the team oversee the planning of repatriation or onward pathway plans for those assessed when appropriate.
Oversee the appropriateness of current placements to meet commissioning and individual needs; including care, quality, and financial outcomes.
Ensure that all care packages and placement proposals are clinically appropriate, have all agreed documentation in place and make best use of agreed resources.
OTHER DUTIES
The post holder will be required to use a computer, either a stand-alone or as part of a networked system and will be responsible for the quality of information. The amount of time spent on this type of work will depend on the job.

Use expert clinical knowledge to support clinical commissioners to agree/negotiate specialist placements or packages of care in response to recommendations made by mental health, learning disability or autism professionals from a variety of providers with respect to eligibility for funding and individual packages of care.
Act as a source of advice on providers with the relevant capabilities and competencies to deliver the agreed care requirements.
Ensure that “responsible commissioner” or “ordinary residence” is established for each individual case in line with national frameworks/guidance, leading on the resolution of disputes in such cases where necessary.
