The Here4You Access Team operates Monday to Friday between the hours of 9am and 5pm.The Here4You Access Team support Lincolnshire CYP Services by staffing the Here4You advice and referral line which provides support, guidance, and self-referral for CYP and families in Lincolnshire as well as advice to professionals of Lincolnshire who work with Children and Young People. The Here4You Access Team screens all incoming referrals to ensure CYP access the right support, at the right time.

The team works remotely, based at our home addresses but with the expectation to travel around the county of Lincolnshire occasionally.

We require an excellent communicator who has knowledge of Children and young people's services in Lincolnshire.

The Here4You Access Practitioners will be instrumental in ensuring that the children and young people of Lincolnshire are directed to the most appropriate service to assess their emotional wellbeing and mental health difficulties.

They will screen all referrals made to CYP services. They will provide triage calls to CYPs and their families if required to ensure they receive the right assessment, at the right time.

They will provide advice and signposting to professionals calling the H4U line and ensure that all signposting information is current and accurate.

Team working
- Provide clinical support to the team based on knowledge and experience as and when needed.
- Supporting the induction of new staff.
- Attendance at team meetings and away days where necessary.
- Completing audits as required (for example quality audits and examining data).
- Support the implementation of service improvements & changes within the team setting.
- Act as Practice Supervisor or Assessor (registered nurse) for student nurses in the team.

Clinical
- Screening referrals to CYP services and liaising with peers to ensure the young person receives the right care at the right time.
- Providing cover on the Here4You line to triage calls and provide support to professionals and parents/carers.
- Completing thorough and holistic triage and risk assessments where necessary.
- Liaise with other professionals/services re. referrals and according to CYP needs.
- Recognition of own skills and professional capability to escalate cases as necessary.
- Gathering feedback from children, young people, parents/carers and other professionals to enable service growth and development.

Administration
- Proficient use of the clinical system to upload and locate demographic information as well as clinical information where appropriate.
- Ensuring accurate and appropriate information is recorded within the young person’s clinical notes.
- Robust risk assessing and triage including ensuring all information is accurate, timely and relevant to CYP care.
