An exciting opportunity is available for a dynamic individual to work within:
Lincolnshire Community Health Services NHS Trusts (LCHS) Integrated Cardiology Service.

The post holder will plan, organise and deliver Cardiac Rehabilitation programmes as agreed with supervising clinician in line with clinical governance and professional standards, working closely with all members of the multi-disciplinary team, seeking opportunities to maximise a patient’s early rehabilitation by implementing and evaluating exercise therapy intervention as per treatment plan for patients with cardiac conditions, thereby providing effective evidence-based care for patients referred to the service under the indirect supervision of qualified clinicians.

To use personal effectiveness and knowledge to contribute to the development and delivery of a high-quality, cost-effective service and support team/service targets which directly contribute to the provider services agenda.

You must be able to travel across the County to locations not served by public transport.

The post holder will participate as a member of the multidisciplinary team in the assessment, planning, delivery and evaluation of patients/clients health and wellbeing needs and participate in the delivery of complex care as prescribed by the qualified practitioner

To organise and run exercise classes in the community and on virtual platforms with the registered staff, maintaining high professional standards.

To be able to use identified assessment tools and outcome measures in line with agreed competency.

To have knowledge of specific physical, emotional, psychological, social, cultural and spiritual needs of the service users and carers and, taking these into account, assist and implement appropriate action to meet those needs

Plan and progress patient interventions with autonomous assessing and planning on a daily basis with indirect supervision of qualified practitioner

Understand a range of non-routine work procedures and practises which require intermediate level of theoretical knowledge and physical skills gained through relevant training and experience

Teach patients/carers with the indirect supervision of a qualified practitioner, and demonstrates educative support to others, e.g. participate in induction of new staff.

Demonstrate working knowledge of other provider’s resource and referral systems to ensure patient needs are met, within scope of practice.

To be actively involved in team effectiveness by attending staff meetings and contributing as a team player to team development. Contribute to the non-clinical day to day running of the service as necessary to ensureitssmooth running.

Promote a positive image of the organisation, department and service.
