A new opportunity has become available for an Out of Area Mental Health Practitioner to join our Bed Management Team. The post holder will be responsible for the co-ordination of care needs, risk assessment and planning for individuals (including their carers) who are placed in out of area inpatient rehabilitation and forensic beds. The post holder will be responsible for improving communication between local and out of area services and ensuring open dialogue and involvement with the patients and their carers.

The post holder will co-ordinate a specified caseload of current out of area service users in inpatient rehabilitation and forensic settings, ensuring that reviews are attended and reports completed. The post holder will also be expected to be the point of contact for the inpatient unit's MDTs, working with them collaboratively. This includes working with the out of area inpatient teams to identify inpatient and/or community pathways in Lincolnshire to reduce length of stay in out of area placements and promote care in the community.

The role will require country-wide travel to ensure a high level of service is provided to Lincolnshire citizens who are placed out of area.
