An exciting opportunity has arisen in the Homehealth Service, this service is a partnership service between South Lincoln Healthcare GP Federation and Lincolnshire Community Health Services.

Homehealth support around 500 residents across 15 care homes. The service is designed to enhance the high-quality service offered by GP practices in the area, by providing a dedicated team of healthcare professionals to work in local care homes. This service is led by GPs but by using the specific expertise and skills of a variety of other professionals, this ensures that each resident’s health and care needs can be met.

**The team comprises of**: - Advanced Clinical Practitioners, Registered nurse, Occupational therapist, Speech and Language Therapist.

The team provide a proactive approach to managing acute and chronic health and well-being issues in care homes, supported by a weekly MDT that includes therapists, mental health, clinical pharmacists, and care home representatives. Residents benefit from personalised approach taken to optimise their health and wellbeing. The registered GPs are an integral part of the team, providing expertise and advice when needed.

The post holder will be responsible for service delivery within Primary Care. Having highly developed specialist, condition specific clinical knowledge and skills and provide autonomous clinical appraisal and evaluating skills, interpreting complex information to form a diagnosis, planning and implementation of evidence-based care in the context of working practice.

Working as an Advanced Clinical Practitioner the post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care demonstrated through the LCHS Advanced Clinical practice framework.

The team are an expert resource, providing a visible clinical profile, having both management, advisory and clinical in-put into the running of the service.

We are looking for people who can demonstrate the right values and behaviours and have excellent leadership and communications skills. Portfolio evidence of core capability across all four Pillars of Advanced Practice is expected.

As part of a team, you will work as an autonomous practitioner, with support from Advanced Clinical Practitioners and GP’s.

Job responsibilities

Core competencies include holistic assessment, history taking, clinical assessment, diagnosis and management, telephone triage, understanding of dementia, nutritional support, referral to and MDT working, palliative and end of life care, advance care planning, long term condition management, referral for investigations, tissue viability, non-medical prescribing.

The vacancy is for a full-time role. The service runs Monday to Friday, 08:00-18:30, not including bank holidays.
