Lincolnshire Virtual Wards are seeking an experienced, dynamic, resilient and compassionate leader to join their team as Clinical Team Lead.
The CTL role requires robust and resilient senior leadership skills that ensure the workforce is empowered to deliver a continued standard of high quality care and great partnership working with the Primary care network and with our internal and external stakeholders.
To apply, you must hold a Registered Nurse or Allied Health Care Professional qualification and registration but it is essential to have a good understanding of community nursing services and processes currently in place. In addition, you must hold a Masters qualification or be able to demonstrate equivalent experience at this level, with evidence in your portfolio of leadership, development and change

To provide day to day hands-on visible clinical management and leadership of an integrated community team.
The post holder is responsible for operational delivery of a designated service portfolio and is accountable for providing the assurance against delivery including quality and performance management and Board Assurance Frameworks.
To support Virtual ward team to provide a patient centred service based on a strong clinical leadership that has high visibility in clinical practice.
To work with the Deputy Divisional Lead to provide effective quality and governance assurance in the integrated community teams you manage.
To maintain compliance with and development of internal and external policies, procedures and guidelines.
To facilitate the integration of physical health, social wellbeing and mental health services locally, alongside the development of wider partnership working within the PCN
To work collaboratively with Clinical Team Leads across LCHS to ensure the delivery of equitable and seamless services for patients.
To influence, lead, support and motivate staff to maintain morale in difficult and challenging situations.
To provide clinical leadership to the entire team by modelling the professional attitudes and behaviours outlined in the LCHS Behaviours Framework.
To be responsible for a delegated budget and contribute to the delivery of the cost improvement programme.
To maintain compliance with and development of internal and external policies, procedures and guidelines.
Ensure that your own skills as a practitioner are developed and maintained to ensure clinically safe practice is delivered at all times.
