This is an exciting opportunity for a committed, passionate and enthusiastic individual to join our transitional oncology team specialising with urology cancer patients. This role is expected to be Trustwide, the base can be dependent on the candidate.

We are looking for a candidate to provide a specialist nursing service to oncology patients transitioning to palliative treatment plans for urology cancer. You will work closely with the MDT, Cancer Care Co-ordinators, community and acute teams. Compassionate and personalised care plans will be required to ensure physical, emotional, social and spiritual needs of patients and their loved ones are met.

Supporting difficult conversations around advanced care planning, decision making and symptom management. Co-ordinating patient care with a focus on their transitional palliative needs.

As a CNS we offer a variety of services to support your development and well- being, we hold quarterly CNS forums, offer clinical supervision and have a wide range of educational opportunities. This role will be a job share.

Main duties of the job

To provide a professional specialist nursing support service to patients transitioning through palliative treatment plans for urology cancer with oncology, offering compassionate, sensitive care where physical, emotional, social and spiritual needs are met.

To utilise specialist knowledge and skills within the Oncology multi-disciplinary management team to support high quality of care and treatment options for patients with a palliative cancer diagnosis.

To support patients undergoing treatment as an inpatient, day case or outpatient.

To utilise clinical expertise to co-ordinate care across providers supporting the treatment of patients from point of entry, through diagnostic studies, diagnosis and treatment plan for cancer through survivorship.

To facilitate communication between primary, secondary and tertiary care providers in support of patient pathways.

To provide specialist education to other staff / students.

To undertake and promote research and clinical audit within the palliative / oncology service.

To develop and implement clinical guidelines and standards.

To be a clinical expert and able to initiate, lead and teach others in the implementation of innovative practice.

Qualifications

Essential


  • Registered General Nurse
  • First level degree or equivalent
  • Post registration qualification in Palliative Care / Oncology
  • Recognised teaching qualification
  • Advanced communication skills training
  • Level 2 Psychological Support Training or willingness to undertake
  • Evidence of continued professional development
  • Demonstrated ability to study at Masters level

Desirable


  • MSc or willingness to undertake
  • Counselling course

Previous Experience

Essential


  • Evidence of extensive post registration experience, including oncology/palliative care nursing at a senior level Band 6 or above
  • Experience of multi-disciplinary team working
  • Leadership and management experience
  • Experience of teaching within the clinical field

Desirable


  • Previous experience of working as a Clinical Nurse Specialist
  • Experience of working as an autonomous practitioner
  • Experience of undertaking clinical audit
